
Senior Patient Advocate Specialist

As a Care Coordination Support Officer, you will provide administrative and co-ordination support to the Care Coordination team with a continual focus on improving process and delivering exceptional customer service.
You will work closely with our clients, their families, carers, and advocates to ensure all clients have a Care Plan that reflects their individual care and service needs.
Key Responsibilities:
- Providing administration support to the Care Coordination team
- Assisting with client intake, assessment, and reviews
- Guiding new and potential clients to easily access services
- Establishing service and equipment requests
- Working collaboratively with the team to assist with the coordination of service for our clients
Requirements:
- A genuine commitment to helping people retain their independence in their own home
- Sensitivity to the needs of older people from different social, economic, and cultural backgrounds
- The ability to use your initiative
- The ability to work independently but also within a team

Additional Requirements:

Preferred Qualifications:
- A current Police Check
- Diploma in Community Service Coordination or Certificate 3 in Aged Care or equivalent
- Experience and/or knowledge of Home Care Packages and Commonwealth Home Support Program
