Job Title: Community Health Nurse

This role involves delivering primary health care to older adults in their homes and community settings. The successful candidate will draw on best practice nursing procedures to provide holistic, person-centred care.

Key Responsibilities:
  • Providing direct clinical nursing care to clients in their homes.
  • Conducting clinical assessments and developing nursing care plans.
  • Working as part of a multidisciplinary team to deliver coordinated care.
Requirements:
  • A genuine passion for providing high-quality care to the elderly.
  • Current AHPRA registration and Bachelor of Nursing or similar qualification.
  • Minimum 3 years postgraduate nursing experience.
