As a specialist case manager, you will provide intensive support to young people at risk and residing in family-based settings or home-based care placements. Your role involves providing assertive outreach and consulting with stakeholders to help individuals meet their goals.

Your key responsibilities include:

  • Managing a caseload of high-risk youth involved with Child Protection;
  • Conducting assessments and safety planning to identify risks and coordinate safety plans;
  • Developing and implementing case plans that promote positive outcomes for clients;
  • Engaging with vulnerable and at-risk individuals and their families using active engagement strategies;
  • Collaborating with relevant stakeholders to ensure the needs of clients are met.

You will also be required to:

  • Document case management tasks and maintain accurate records;
  • Generate formal reports on client progress and outcomes;
  • Work collaboratively with colleagues to achieve shared goals and objectives.
