About the Role

For our Family Based Care team servicing the Wollongong & Illawarra region, you will work closely with children, young people, their families, and carers to develop and execute effective strategies that promote stability and growth.

  • Key Responsibilities:
  • Create and implement individualized case plans that address the unique needs and circumstances of each child or young person
  • Collaborate with families, carers, and other stakeholders to ensure a cohesive and supportive environment
  • Monitor progress and make adjustments as necessary to ensure optimal outcomes

Requirements:
