About the Role:
This is a dynamic opportunity to make a positive impact on the lives of children and young people.
We are seeking an experienced Case Manager to join our team, who will work collaboratively with families, carers, and communities to create safe and supportive environments where children can thrive.
The successful candidate will be responsible for creating and implementing case plans that strive towards achieving permanency goals, building strong relationships with children, their families, and carers.
Key Responsibilities:
- Collaborate with children, young people, families, and carers to develop and implement effective case plans.
- Build and maintain strong relationships with key stakeholders to achieve optimal outcomes.
- Provide guidance and support to carers in their foster care journey.
Required Skills and Qualifications:
- A relevant bachelor's degree in Social Work, Social Welfare, Psychology, or a similar field.
- Previous experience in case management within an Out of Home Care/Child Protection setting.
- Experience supporting children and young people with disabilities, trauma, abuse, and neglect.
