About the Role

This is an ongoing full-time opportunity based in Griffith/Wagga Wagga, NSW. We're seeking a compassionate and dedicated individual to join our team as a Way Back Support Coordinator. In this role, you'll provide critical support to individuals in the first three months following a suicide attempt, helping them access non-clinical care and practical resources.

Key Responsibilities:
  • Deliver non-clinical care and practical support to individuals and their families, working collaboratively with participants to develop and review personalized well-being plans.
  • Support engagement in intake and assessment processes, liaising with hospitals and agencies as necessary, and facilitate connections to health, clinical, and community-based support services.
  • Maintain accurate electronic records and documentation of participant activities, upholding participant rights, confidentiality, and self-determination while ensuring safety planning.
  • Build and maintain relationships with key referrers and consortium partners, participating in monitoring, reporting, and evaluation activities to achieve program objectives.
What You Need:
  • A relevant qualification or commensurate experience in working in a support role for people with a mental illness, generally a Certificate IV qualification or above.
  • Experience in working with people presenting with suicide thoughts or behaviours and who have experienced trauma.
  • A current valid Driver's License and the ability to undertake travel for the role.
  • The right to work within Australia.
  • Satisfactory Police Records Check and Working with Children Check prior to commencement.
  • NDIS Worker Orientation Module Certificate and NDIS Worker Screening Check prior to commencement.
