About the Role:
We are seeking a highly skilled and compassionate Lead Care Professional to lead our care team in delivering exceptional care to our residents. As a key member of our team, you will be responsible for ensuring that our residents receive the highest standard of ongoing assessment, care planning, and evaluation.
Key Responsibilities:
- Oversee daily clinical operations to ensure residents receive best possible care.
- Support and lead the care team, including coaching, mentoring, staff allocations, delegation, performance management, and education.
- Review clinical incidents, complaints, and changes, providing solutions to reduce risks.
- Lead compliance with AN-ACC assessments.
- Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
- Foster a strong team environment.
- Prepare and conduct clinical governance audits.
- Provide insights and innovative solutions for continuous clinical improvement.
- Oversee medication management according to resident needs and legislative requirements.
- Liaise with allied health, medical practitioners, and other services to provide optimal resident care.
Requirements:
To be successful in this role, you will need:
- Bachelor of Nursing with current APHRA registration.
- Postgraduate qualification in aged care (highly regarded).
- Strong leadership, communication, and interpersonal skills.
- A person-centred care mindset.
- A passion for nurturing a strong team environment.
- Ingrained passion and empathy for residents.
- Proven track record in delivering quality clinical care within an aged care or acute setting.
- Excellent clinical assessment and triaging skills, as well as strong care planning capabilities.
- Proven experience in fostering a reporting culture balanced with effective investigative skills.
- An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes.
