
Nurse Advocate

As a Registered Nurse in our community team, you will provide high-quality nursing service that meets the needs of our clients. Your role will involve delivering healthcare outside of traditional hospital settings.
Our nurses play a vital role in ensuring the care service delivery is undertaken in line with our values and policies.
- Assessment, planning, implementation, and evaluation of nursing care tailored to individual needs.
- Monitoring vital signs (e.g., blood pressure, temperature, pulse) and responding to changes in health status.
- Administering treatments such as injections, wound care, and medication management.
- Supporting chronic disease management, including diabetes, dementia, and continence care.
- Providing palliative care and support for patients with life-limiting conditions.
- Working with multidisciplinary teams, including doctors, physiotherapists, and social workers.
Responsibilities
- Develop and implement effective care plans that meet the unique needs of each client.
- Monitor and report on client progress, identifying areas for improvement and implementing changes as needed.
- Collaborate with other healthcare professionals to ensure comprehensive care is provided to clients.
- Provide education and support to clients and their families on health-related issues.
To be successful in this role, you will require:
- Current Nurse Registration with AHPRA.
- Sound knowledge of current clinical practice.
- Knowledge of the Aged Care Quality Standards.
- Ability to work effectively in a team environment.
- Excellent communication and interpersonal skills.
- Strong commitment to person-centred care.
- Empathy with the needs of the elderly.
- Awareness of WHS requirements, mainly manual handling and infection control.
- Sound knowledge of notification and reporting obligations of Registered Health Practitioners.
- Demonstrated understanding of the requirements of the Privacy and Personal Information Protection Act (NSW).
