Job Opportunity: Community Health Nurse

We are seeking a skilled and compassionate Community Health Nurse to join our team. The ideal candidate will have experience in delivering person-centred care in home and community settings.

The role involves working closely with individuals, families, and multidisciplinary teams to provide holistic support. A strong ability to transfer skills from medical, surgical, rehabilitation, or aged care settings to a community-based model is essential.

The successful candidate will be responsible for expanding their nursing skills across generalist and specialist areas, including wound management, medication management, continence support, PEG tube changes, intercostal drain care, and diabetes education.

Key responsibilities include:

  • Delivering high-quality, person-centred care and advocating for individualised support
  • Working independently and as part of a collaborative team
  • Providing clinical capability across wound, continence, and medication management
  • Supporting clients in the home and understanding palliative care principles

The ideal candidate will have a strong commitment to CORE values of Collaboration, Openness, Respect, Empowerment, and lifelong learning.

Requirements include current AHPRA registration and a current NSW Driver's Licence, with the ability to work across Newcastle and Raymond Terrace regions.
