We deliver comprehensive care to over 210,000 patients nationally in a leading healthcare setting. Our innovative model of care relocates patients from hospital beds to a city hotel, where they can rest and recover in comfort at no cost, freeing up beds for those who need them.

We are seeking a full-time Social Worker to deliver care to patients with varying complexity on a fixed-term contract. Based in the Pullman Hotel, Adelaide, you will work in a dynamic environment, utilizing cutting-edge technology. Your expertise will contribute to achieving exceptional health outcomes for sub-acute patients.

Key Responsibilities:

  • You will collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.
  • You will facilitate group sessions and contribute to health promotion activities as required.
  • You will provide effective assessment and triage, timely referrals, accurate information, coordinated care, and prompt follow-up.
  • You will advocate for patients' needs and coordinate access to services.
  • You will participate actively in clinical review meetings and continuous professional development.

About You:

We are looking for someone passionate about innovative models of care and excited by this dynamic way of working. Additionally, you have:

  • A degree in Social Work, with eligibility for membership with a relevant professional association.
  • Demonstrated experience in a sub-acute or similar healthcare setting.
  • Strong communication, advocacy, and problem-solving skills.
  • Excellent interpersonal skills, with the ability to adapt to evolving service needs.
  • Proficiency in Microsoft Office and other relevant documentation systems.
  • You hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain them).
  • If successful, you are willing and able to provide proof of up-to-date vaccinations.
