Staying Deadly|headspace Deception Bay seeks a dedicated Care Coordinator to support the mental health, wellbeing and social needs of Aboriginal and Torres Strait Islander young people aged 12-25.

About the Role

The successful candidate will work in a multidisciplinary team to provide outcome-focused health, social and practical support to clients. They will manage a caseload of young people with varying needs, empowering them through intensive case management, planning and coordination.

Day-to-Day Duties
  • Engage with the community to raise awareness of Staying Deadly|headspace and promote mental health and wellbeing.
  • Develop and maintain support plans in conjunction with clients and relevant service providers.
  • Work with clients in a trauma-informed approach to formulate, implement and review well-being plans.
  • Participate in care coordination activities to fulfill needs of assigned clients transition care plans.
About You

To be successful in this role, you must be an Aboriginal or Torres Strait Islander person.

  • Current C Class Driver's Licence (QLD)
  • Current Criminal History Check
  • Valid Blue Card
  • Current CPR and First Aid Certificate
  • Certificate IV or Higher in Aboriginal and/or Torres Strait Primary Health or willing to complete
