The opportunity
As a Community Nurse, you'll build rapport with clients, provide care and service in line with their personalized care plans, and work autonomously with a patient-centered approach. You'll have the flexibility to work on a casual basis, ideally on Monday and Tuesday, with some optional weekend work available.

Key responsibilities
Provide health assessments, chronic wound care, and medication administration to clients in their homes
Work with clients to develop personalized care plans and provide education and support to patients and their families
Collaborate with the community team to ensure seamless care delivery

What we're looking for
Current unrestricted AHPRA as a Registered Nurse
Working with children check/DCSI or equivalent
Current drivers' license & reliable vehicle
2 Years' recent acute experience & ability to spot client deterioration
Experience in PICC care/dressing/removal
Chronic wound care experience is a bonus as well as cannulation skills
Competent IT skills/literacy
