Vulnerable Care Specialist

We are seeking a reliable and passionate support worker to provide person-centered, complex care and behavioral support to our participants.
The work involves supporting NDIS participants in Ipswich area. You will be required to access our app for clocking in and out of shifts, providing in-home support, home domestic duties, community access, transportation, reporting, and documentation.
To be eligible for this position, you will need to:
- Work unsupervised with vulnerable participants, assisting them with daily activities to meet their goals as outlined in individual plans.
- Provide consistent support, encourage, and motivate participants to meet their goals and develop independent skills through person-centered practices.
- Support participants with company access events and services.
- Assist individuals to and from community-based activities and medical appointments when required.
- Provide dressing assistance and hygiene needs when required.
- Develop strong relationships with clients through great communication skills.
- Have experience with behaviors of concern (not mandatory).
- Accurately document reports and relevant information.
- Be available and willing to pick up last-minute shifts and sleep overs/active nights when required.
A can-do attitude, enthusiasm, and great communication skills are desirable. Excellent prioritization and organizational skills are also essential.
