Job SummaryWe are seeking a highly skilled and compassionate Family Support Practitioner to join our team. As a key member of our community health team, you will work closely with families to empower them to make effective change and overcome identified challenges.
Key Responsibilities- Develop and implement case management plans to support families in achieving their goals.
- Provide high-quality, culturally sensitive support to families, focusing on building their agency and self-efficacy.
- Collaborate with internal and external stakeholders to ensure seamless service delivery and access to holistic care.
- Conduct client-led assessments and develop Care Plans to identify client goals and needs.
- Provide targeted interventions and facilitate family-facilitated group meetings to achieve client goals.
- Work closely with the whole-of-family, irrespective of primary client, to meet the totality of family needs.
- Current Bachelor's Degree in Social Work, Human Services, or a related field, with 2 years' relevant experience in social services.
- OR Current Diploma in Community Services, Social Work, or a related field, with 3 years' relevant experience in social services and at least 1 year in a case management role.
- Demonstrated ability to work in a performance-driven environment, meeting individual weekly, monthly, and quarterly performance targets.
- Demonstrated knowledge and ability to provide behavior change interventions that build client self-awareness and active change in achieving their identified goals.
- Valid police check, current drivers' license, and Working with Children's Card (Blue Card) or eligibility to apply.
- Permanent full-time position with a starting annual salary of $86,000 (depending on qualifications and experience).
- 11.5% Super + Not for Profit Salary Packaging (increases your take-home pay).
