About the Role
">The NDIS Mentor role is a hybrid position that requires a unique blend of skills and experience. As an NDIS Mentor, you will be responsible for delivering high-quality support work to individuals with disabilities, with a focus on community access and capacity building.
">Key Responsibilities

- Provide direct, person-centred support work to individuals with disabilities, including assistance with daily living activities and implementation of their individual NDIS plans.">
- Deliver support coordination services, including connecting individuals with appropriate supports and services, monitoring plan progress and outcomes, and advocating on their behalf.">
- Maintain accurate and timely records of support provided, in accordance with NDIS requirements and organisational policies and procedures.">
- Work collaboratively with individuals, their families/carers, and other service providers to ensure a coordinated and person-centred approach.">
- Adhere to the NDIS Code of Conduct and organisational policies and procedures.">
Requirements
">To be successful in this role, you will require:
">- ">
- A minimum of Cert IV Disability Support, Community Services, Mental Health, Allied Health Assistance or equivalent qualification.">
- An NDIS Worker Screening Check and Working with Children Check.">
- At least 1 year experience in providing support coordination services to people with disabilities.">
- At least 2 years experience in providing support work services, with a focus on community access and capacity building, to people with disabilities.">
- An ability to support individuals who have a lived experience of mental ill-health.">
- Strong administrative skills, including written documentation and case notes.">
- Strong knowledge and respect of NDIS guidelines and compliance requirements.">
- Intermediate computer skills.">
