About the Role

The NDIS Mentor role is a hybrid position that requires a unique blend of skills and experience. As an NDIS Mentor, you will be responsible for delivering high-quality support work to individuals with disabilities, with a focus on community access and capacity building.

Key Responsibilities

  • ">
  • Provide direct, person-centred support work to individuals with disabilities, including assistance with daily living activities and implementation of their individual NDIS plans.">
  • Deliver support coordination services, including connecting individuals with appropriate supports and services, monitoring plan progress and outcomes, and advocating on their behalf.">
  • Maintain accurate and timely records of support provided, in accordance with NDIS requirements and organisational policies and procedures.">
  • Work collaboratively with individuals, their families/carers, and other service providers to ensure a coordinated and person-centred approach.">
  • Adhere to the NDIS Code of Conduct and organisational policies and procedures.">

Requirements

To be successful in this role, you will require:

  • ">
  • A minimum of Cert IV Disability Support, Community Services, Mental Health, Allied Health Assistance or equivalent qualification.">
  • An NDIS Worker Screening Check and Working with Children Check.">
  • At least 1 year experience in providing support coordination services to people with disabilities.">
  • At least 2 years experience in providing support work services, with a focus on community access and capacity building, to people with disabilities.">
  • An ability to support individuals who have a lived experience of mental ill-health.">
  • Strong administrative skills, including written documentation and case notes.">
  • Strong knowledge and respect of NDIS guidelines and compliance requirements.">
  • Intermediate computer skills.">
