Job Description:

As a key member of the multidisciplinary team, you will provide intensive care coordination for consumers with chronic and severe substance use issues. This role involves working closely with clients to support stabilization, reintegration, and manage underlying cognitive impairments.

The program targets clients with complex drug and alcohol dependence who experience multiple needs. It aims to reduce substance use, prevent hospital admissions related to substance issues, improve access to long-term treatment, enhance physical and mental health, nutrition, social functioning, and reduce relapse risk through prevention strategies.

Required Skills & Qualifications:
  • Applicable vaccination requirements for a Category A role
  • Working With Children Check (WWCC)
  • Current registration as a Social Worker or eligibility through a Social Work degree with AASW
  • Valid NSW Driver's Licence
