Job Role

An opportunity exists for a qualified professional to join our team as a Substance Abuse Counsellor/Case Manager.

This position will involve providing integrated post-discharge outreach and care coordination for clients exiting the Involuntary Drug and Alcohol Treatment program. The successful candidate will support recovery through community linkages, client education, and continuous service improvement within a collaborative, quality-focused environment.

Where You'll Be Working

Northern Sydney Local Health District - St Leonards

Key Responsibilities
  • Provide assertive outreach case management for clients using drugs and/or alcohol, including referral to inpatient/outpatient drug and alcohol treatment, home visits, transporting clients to/from medical appointments, housing and income support, NDIS applications, referrals for community-based supports, liaison with medical teams, and case coordination.
Candidates Will Need To Meet The Following Criteria
  • Current registration with the relevant discipline's governing body - Psychology, Occupational Therapy, or Counselling with current registration in the Counsellors Association of Australia (PACFA), or eligibility for Membership with the AASW. For Health Education Officer Graduates, relevant tertiary qualifications in health promotion, public health, or other behavioural health sciences.
  • Demonstrated experience and commitment to professional and ethical boundaries in consultation with a medically led multi-disciplinary treatment team.
  • Knowledge and understanding of the social and emotional needs of patients with drug and alcohol issues, including clinical response and management of trauma, grief, loss, domestic and family violence, and child protection.
