Job Role: Therapeutic Case Manager

 

  • We are seeking a highly skilled and experienced THERAPEUTIC CASE MANAGER to join our team in delivering best practice statutory casework services to young people with minimal supervision using high-level assessment and intervention skills.
  • The successful candidate will have the ability to deliver integrated and culturally appropriate services, preparing and presenting information and evidence to the Children's Court, including conducting pre- and post-court interviews, liaising with key court stakeholders, and preparing reports and other documentation as required.
  • This is an exciting opportunity for a qualified professional to engage with and establish partnerships with internal and external stakeholders and service providers to achieve quality casework outcomes.
  • You will be responsible for provision and effective coordination of the therapeutic alliance, formulating a singular case plan with consistent action-oriented goals to support the young person and their family members.
  • This role requires you to undertake a range of record and information management tasks, including maintenance of case records, preparation of reports, and data entry to ensure casework and accountability in accordance with policy and procedures.
  • Liaising with Youth Justice Service Centre staff in relation to complex cases, development of programs, exercising of statutory delegations, payment of program-related costs, and other matters as required.
  • You will also actively participate in and deliver training and professional development activities to develop, maintain, and enhance knowledge and competencies to ensure that service delivery is based on contemporary, evidence-based, best practice principles.
  • Actively participating in regular formal and informal supervision, applying self-reflective practices, and contributing to peer supervision.
  • This role involves a leadership and mentoring role to less experienced staff, including supervision of Youth Workers involved in the Integrated Case Management Program.

  • Assist young people and their families in obtaining employment, education, accessing health services, safe housing, and learning responsible parenting skills.
  • Demonstrate cultural capability and/or a commitment to developing cultural capability to ensure an inclusive culture that supports departmental programs and community partnerships aimed at preserving cultural connections for Aboriginal and Torres Strait Islander peoples.
