Care Coordinator Job Description

As a Care Coordinator, you will be the key to connecting individuals and communities with the resources they need to thrive. Our team is dedicated to empowering people to live healthier, more fulfilling lives.

The Care Finder program aims to provide specialist and intensive assistance to help people in targeted population groups understand and access aged care services and connect with other relevant supports in the community.

Key Responsibilities:
  • Practice assertive outreach and explore ways to engage with people in targeted populations, connecting them to services within the community.
  • Use a person-centred approach to provide specialist and intensive assistance to senior Australians who need intensive support, helping them understand and access aged care services and connect with other relevant supports.
  • Build relationships within the community to promote the Care Finder program and assist with reaching potential clients.
  • Support clients to understand and connect with required services, including My Aged Care, using warm referrals, and monitor client engagement.
  • Maintain accurate and confidential case notes, participate in reporting.
Requirements:
  • Experience working with older adults in the community, with an understanding of available supports and services, and the process to access them.
  • A commitment to delivering a person-centred approach.
  • Understanding of the policy and funding environment of the aged care and community services sector, along with a working knowledge of current legislation.
  • Ability to work collaboratively with other services to ensure integrated delivery.
  • A strong understanding of, and commitment to, cultural safety and trauma-informed care.
Qualifications and Requirements:
  • Relevant qualifications (e.g. Social Work, Human Services, Aged Care, Community Services or Health) and/or relevant experience.
  • Current Australian driver's licence, registered vehicle, insurance.
