
Community Support Specialist

We are seeking a highly skilled and compassionate Community Support Specialist to join our team. As a key member of our community-focused organization, you will play a vital role in connecting individuals with meaningful services and activities that enhance their sense of wellbeing and belonging.
The successful candidate will have a tertiary degree in Social Work, Psychology, or a related field, and a minimum of two years of relevant community work experience. You will possess excellent clinical reasoning, problem-solving, and communication skills, as well as the ability to work autonomously while contributing to a collaborative team environment.
Your expertise will be utilized to develop and maintain relationships with internal and external stakeholders, including medical professionals, government and non-government agencies, and community organizations. You will also be responsible for identifying and implementing continuous quality improvement opportunities aligned with our values.

About the Role:
- Provide client-centred support through strengths-based intake and assessment, and case management.
- Assist clients in achieving personal health and wellbeing goals by connecting them with community-based groups, activities, services, and supports.
- Manage comprehensive client assessments and case management, ensuring all interactions are planned and structured to improve health, connection, and well-being outcomes.
- Maintain accurate and comprehensive records, including reports for various stakeholders.
- Engage actively with the community, promoting Social Prescribing and fostering social inclusion and belonging.
- Develop and maintain relationships with internal and external stakeholders.
- Identify and implement continuous quality improvement opportunities aligned with our values.
Selection Criteria:
- Tertiary Degree in Social Work, Psychology, Human Services, Public Health/Health Promotion, or a related field.
- Minimum of two years of relevant community work experience.
- Strong understanding of evidence-based frameworks, including person-centred and trauma-informed approaches.
- Demonstrated ability to work with disadvantaged and vulnerable individuals or those facing significant barriers.
- Excellent clinical reasoning, problem-solving, and communication skills.
- Experience in developing and maintaining relationships with various stakeholders.
- Ability to work autonomously while contributing to a collaborative team environment.
