About This Role:

This position is responsible for leading patient-centred assessment and discharge planning for individuals admitted to sub-acute rehabilitation and GEM programs.

Main Responsibilities:
  • Develop effective communication strategies with patients and families, establish clear goals tailored to individual needs, and ensure timely discharges.
  • Manage a caseload of clients with complex needs, support them in achieving their objectives, and liaise with community services to deliver comprehensive care.
  • Promote collaboration within multidisciplinary teams, enhance care coordination capabilities, and contribute to access and flow strategies across the unit.
