Rehabilitation Care Specialist

We are seeking a motivated Rehabilitation Care Specialist to lead patient-centred assessment and discharge planning for individuals admitted to our sub-acute rehabilitation programs.

Main Responsibilities
  • Communicate effectively with patients and families, establish SMART goals tailored to individual needs
  • Ensure timely discharges through coordinated efforts
  • Manage a caseload, support clients with complex needs
  • Liaise with community services and stakeholders to deliver comprehensive care
Leadership Role

As a leader within the multidisciplinary team, you will play an active role in mentoring staff, enhancing care coordination capabilities, and contributing to access and flow strategies.
