
Cohesive Care Coordinator

We are seeking a skilled and dedicated Care Coordinator to lead patient-centred assessment and discharge planning for individuals admitted to our rehabilitation programs.
The successful applicant will be responsible for providing effective communication with patients and families, establishing SMART goals tailored to individual needs, and ensuring timely and coordinated discharges. They will manage a care coordination caseload, support clients with complex needs, and liaise with community services and stakeholders to deliver comprehensive care.
