Senior Aged Care Coordinator

Seeking an experienced Care Management Professional to lead our team in delivering exceptional care and support services.
Key Responsibilities:
- Develop, implement, and review client care plans, ensuring alignment with individual goals and government regulations.
- Establish Home Care Agreements and maintain accurate compliance documentation.
- Foster strong relationships with clients, their families, advocates, staff, funding bodies, doctors, and allied healthcare professionals.
- Enable clients to make informed purchasing decisions within their Home Care package allocations.
- Promote process improvement and facilitate the implementation of requested changes.
- Contribute to the growth and development of the Home Care Package program.
Requirements:
- Commitment to delivering high-quality service to clients.
- Experience in aged care, including care planning processes, assessments, and goal setting.
- Excellent communication and problem-solving skills, with a customer-focused approach.
- Relevant qualifications or experience in nursing and/or community services & frontline management.
- Ability to manage time effectively in a dynamic environment with changing priorities.
