
Cognitive Care Specialist

We are seeking a skilled and compassionate support worker to join our team in providing person-centered care to individuals with cognitive impairment and end of life needs.
Key Responsibilities:- Providing direct support to individuals with complex care needs.
- Implementing tailored support plans designed to meet the unique needs and goals of each client.
- Managing challenging behaviors, including aggression and self-harm, in a safe and effective manner.
- Assisting clients with daily living activities, personal care, and community participation.
- Building positive relationships with clients, their families, and other support networks.
- Documenting client progress, interventions, and incidents in accordance with NDIS guidelines.
- Minimum of two years experience working as an NDIS Support Worker with a focus on cognition and complex care needs.
- Demonstrated understanding and experience in supporting individuals with autism and non-verbal communication.
- Proven ability to manage challenging behaviors in a calm and professional manner.
- Familiarity with NDIS documentation requirements.
- Certificate III or IV in Disability, Individual Support, or a related field.
- Current First Aid and CPR certification.
- Excellent communication and interpersonal skills.
- Strong problem-solving skills and adaptability.
- A compassionate approach to care and support.
