The Sustaining NSW Families nurse-led home visiting program offers vital support to vulnerable families from pregnancy until their child is two years old.
This leadership role requires a skilled Child and Family Health Nurse with expertise in managing new referrals, undertaking initial client engagement and clinical assessments, providing clinical consultation, and leading case reviews.
- Manage new referrals to the SNF Service and intake
- Undertake initial client engagement and clinical assessments
- Provide clinical consultation for team members
- Lead case reviews
To be successful in this position, candidates will need to meet the following criteria:
- Be a Registered Nurse-Division 1 with Australian Health Practitioner Regulation Agency (APHRA)
- Possess relevant management tertiary qualification or equivalent work experience
- Hold extensive post-registration experience in Child & Family Health Nursing (CFHN)
- Demonstrate experience working with vulnerable families and a clear understanding of early intervention
- Have experience in managing a team and knowledge in strategic and operational service planning and quality improvement processes
