Job Opportunity

As a skilled healthcare professional, you will provide high-quality child and family health services to families caring for children 0-3 years of age within a community-focused setting.

  • Work collaboratively with a multidisciplinary team to contribute to evidence-based practice and care delivery.
  • Utilize best-practice screening and assessment tools to identify health variations or family vulnerabilities and develop tailored care plans that meet family goals.
  • Build parental confidence and skills through education, resources, guidance, and role modelling safe and responsive care practices.
  • Apply a strength-based approach to work in partnership with families, promoting optimal child and family health outcomes.
Key Responsibilities:

The successful applicant will be responsible for:

  • Conducting comprehensive child and family health assessments and providing interventions to address identified needs.
  • Collaborating with healthcare professionals to ensure continuity of care and effective communication.
  • Developing and implementing individualized care plans that reflect family goals and priorities.
  • Maintaining accurate and up-to-date records of client interactions and care plans.
  • Participating in ongoing professional development to stay current with best practice and emerging trends in child and family health nursing.
