About the Role

We are seeking a highly motivated and compassionate Disability Support Worker to join our team in Rockhampton. As a key member of our team, you will be responsible for providing day-to-day care and supervision to our clients, as well as facilitating community access and transport. Your strong communication and interpersonal skills will enable you to build positive relationships with our clients and their families.

Key Responsibilities

  • Provide day-to-day care and supervision to clients, including personal care and assistance with daily activities.
  • Facilitate community access and transport, ensuring our clients can participate in community activities and maintain their independence.
  • Monitor and maintain Person Centred Plans, tracking client progress and identifying areas for improvement.
  • Develop and maintain positive relationships with clients and their families, ensuring their needs are met and their goals are achieved.

Requirements

  • Unrestricted Australian Drivers Licence and access to a reliable vehicle.
  • Current Provide First Aid and CPR Certificate.
  • Prior experience in the disability sector, with a focus on supporting clients with complex needs.
