Care Coordinator for Vulnerable Families
As a skilled Care Coordinator, you will play a pivotal role in supporting vulnerable families and children. This position offers a unique opportunity to make a positive impact on the lives of those who need it most.
About the Role:
This is a challenging yet rewarding full-time opportunity that involves coordinating all aspects of referral, entry, engagement, and allocation of cases from Child FIRST/The Orange Door.
You will undertake risk and needs assessments, deliver short-term case management, and provide an active holding function to children and families who are referred to the Family Services Program.
Working collaboratively with Child Protection, you will support families to enhance parenting capacity, family functioning, and promote child safety.
Key Responsibilities:
- Coordinate referrals, entries, engagements, and allocations of cases
- Undertake risk and needs assessments
- Deliver short-term case management
- Provide an active holding function to children and families
Requirements:
To be successful in this role, you will require a tertiary qualification in Social Work, Psychology, or a related discipline, along with current driver's license valid in Victoria.
Demonstrated experience in the community services sector, particularly in case management, is essential.
You should also have experience working with young people who have experienced abuse and trauma, as well as an understanding of the implications of abuse and trauma.
