Senior Care Coordinator

As a Senior Care Coordinator, you will be responsible for delivering exceptional care and support to our clients in the comfort of their own homes. You will work closely with our team to develop personalized care plans that meet the unique needs and preferences of each client.

Key Responsibilities:

- Work collaboratively with the care team to ensure seamless delivery of care services.
- Monitor and evaluate client progress, identifying areas for improvement and implementing changes as needed.
Requirements:
- 10 years of experience as a Registered Nurse or equivalent.
- Strong background in healthcare, with a focus on senior care.
- Excellent communication and interpersonal skills.
- Ability to build strong relationships with clients and their families.
- Highly organized and able to prioritize tasks effectively.
- Proficient in Microsoft Office Suite and other relevant software applications.
