Job Description

We are seeking a dedicated professional to join our team as a Domestic Assistance and Home Meals Project Officer. This role involves coordinating and administering domestic assistance and home meals services in line with CHSP guidelines.

The successful candidate will be responsible for ensuring smooth program operations, delivering high-quality client-focused service, and contributing to community outreach and administrative support.

Key Responsibilities
  1. Manage client referrals via the My Aged Care Provider Portal and support the intake process for CHSP – DA and Home Meals services.
  2. Conduct assessments and develop Support Plans based on individual client needs, preferences, and funding eligibility.
  3. Deliver menus, assist with meal selections, and support clients in accessing food support and other community resources.

Additional responsibilities include:

  • Coordinate the delivery of Domestic Assistance and Home Meals services in accordance with CHSP guidelines and support plans.
  • Collaborate with Rostering teams, Contractors, and Meal vendors to ensure accurate, timely, and uninterrupted service provision.
Required Skills and Qualifications

To be successful in this role, you will require:

  • A relevant qualification in Community Services, Community Development or Aged Care, or similar discipline.
  • Demonstrated experience in providing high-quality customer service.
  • Experience of working with clients from diverse backgrounds.
  • Relevant knowledge and experience in CHSP, HCP, NDIS services or other community services.
