Home Care Speech Therapist Specialist

We are seeking a highly skilled and compassionate Speech Therapist to join our team.
About the Role:As a Speech Therapist, you will be responsible for providing high-quality care to patients in their homes and communities. You will work closely with patients, families, and caregivers to develop personalized treatment plans that promote dignity and independence.
Key Responsibilities:- Conduct thorough assessments of patients' speech and communication skills.
- Develop tailored treatment plans that meet the unique needs of each patient.
- Provide expert speech and language therapy to patients in their homes.
- Monitor and document patient progress towards established goals.
- Educate patients, families, and caregivers on effective speech therapy techniques and strategies.
- Participate in collaborative meetings to discuss patient progress and treatment plans.
- Maintain accurate and complete patient records.
- Bachelor's or Master's degree in Speech-Language Pathology.
- Current licensure to practice as a Speech Therapist in the relevant state.
- Minimum of 1 year of experience working with patients in a home care setting.
- Exceptional verbal and written communication skills.
- Able to work independently and as part of a team.
- Excellent time management and organizational skills.
- Able to travel to patient homes within designated areas.
