Job Overview

We are seeking a highly skilled and dedicated case manager to join our team in Launceston with regular travel to Shearwater.

About the Role

This is an excellent opportunity to work in a fast-paced environment, providing day-to-day management of client cases, care planning and provision of consumer-directed services.

  • Manage a client caseload, developing effective care plans that meet individual needs.
  • Build strong relationships with clients, their families and healthcare professionals.
  • Work collaboratively as part of a multidisciplinary team to deliver high-quality care services.
Key Responsibilities
  • Case Management: Provide day-to-day management of client cases, including assessment, planning, implementation and review of care plans.
  • Care Planning: Develop and implement comprehensive care plans that meet the needs of clients, taking into account their goals, preferences and values.
  • Service Delivery: Deliver high-quality consumer-directed services that promote independence, dignity and well-being.
Requirements
  • Relevant experience in case management, community services or aged care.
  • Knowledge of Commonwealth Home Support Program (CHSP) and Veterans' Home Care (VHC) funding.
  • High-level planning and organisational skills, with ability to manage competing priorities.
  • Strong communication and interpersonal skills, with ability to build rapport with clients, families and healthcare professionals.
