Pharmacist Grade 3

The Pharmacy Senior Clinician will play a crucial role in supporting the appropriate and quality use of medicines within the Health Independence Program. As the single program Pharmacist, you will be responsible for receiving referrals from care coordinators and managing a caseload of clients, prioritizing those most at risk. You will work closely with care coordinators and other team members to implement client goals and provide secondary consultation on medications and their management.
Key Responsibilities- Undertake pharmacy-specific assessments, plans, and implements strategies to support quality use of medicines, adhering to best practice principles and evidence-based practice.
- Provide secondary consultation on medications and their management, ensuring effective communication and collaboration with team members.
- Ensure effective management of pharmacy aids and equipment to maximize client independence and safety, minimizing organizational risk.
- Regularly liaise with the SVHM Pharmacy department to support integration and maintenance of professional standards and legislative requirements.
- Provide pharmacy clinical expertise and leadership in clinical and team meetings, promoting a culture of continuous improvement.
- Participate in pharmacy-specific supervision, ensuring ongoing professional development and growth.
- Bachelor of Pharmacy or higher degree.
- Current AHPRA registration, meeting CPD requirements for pharmacists conducting medication reviews.
- Registered (unrestricted) with the Pharmacy Board of Australia.
- Minimum of seven (7) years post-graduate experience.
- Victorian Driver's License.
- Highly developed interpersonal skills, incorporating excellent communication and collaboration qualities.
- Current National criminal history check or willingness to obtain.
