The Pharmacy Senior Clinician plays a crucial role in supporting the appropriate and quality use of medicines. As the single program Pharmacist, you will receive referrals from care coordinators and manage a caseload of clients, prioritizing those most at risk. You will work closely with care coordinators and other team members to implement client goals and provide secondary consultation on medications and their management.
Key Responsibilities- Undertake pharmacy-specific assessments, plans, and implements strategies to support quality use of medicines.
- Provide secondary consultation on medications and their management.
- Ensure effective management of pharmacy aids and equipment to maximize client independence and safety.
- Ensure regular liaison, communication, and consultation with the SVHM Pharmacy department to support integration and maintenance of professional standards and legislative requirements.
- Provide pharmacy clinical expertise and leadership in clinical/team meetings.
- Participate in pharmacy-specific supervision.
- Bachelor of Pharmacy or higher degree.
- Current AHPRA registration meeting CPD requirements for pharmacists conducting medication reviews.
- Registered (unrestricted) with the Pharmacy Board of Australia.
- Minimum of seven years post-graduate experience.
- Victorian Driver's License.
- Highly developed interpersonal skills incorporating excellent communication and collaboration.
- Current National criminal history check or willingness to obtain.
