Senior Aged Care Practitioner

As a Senior Aged Care Practitioner with Infinite Care, you will play a vital role in delivering high-quality, person-centred care to our residents and their families.
Your primary responsibility will be to provide exceptional nursing care, ensuring the physical, emotional, and social needs of our residents are met.
You will work collaboratively with our multidisciplinary team to develop and implement individualised care plans that promote resident autonomy, dignity, and well-being.
Key Responsibilities- Provide comprehensive nursing assessments, identifying areas for improvement and implementing strategies to enhance resident outcomes.
- Develop, implement, and evaluate individualised care plans, prioritising resident goals and preferences.
- Lead and mentor junior staff members, promoting a culture of excellence and continuous learning.
- Collaborate with the interdisciplinary team to address complex care needs, ensuring seamless communication and coordination.
- Maintain accurate and timely documentation, adhering to legislative and organisational requirements.
- Bachelor of Nursing degree or equivalent.
- AHPRA Registration with no restrictions.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
- Ability to work effectively in a fast-paced environment, prioritising tasks and managing competing demands.
- Strong leadership and mentoring skills, with experience in leading and developing teams.
- Knowledge of aged care principles, policies, and procedures, with a commitment to ongoing professional development.
