About the Role

We are seeking a skilled and compassionate Holistic Care Specialist to join our team at Bolton Clarke. This is a part-time permanent role, working 40 hours per fortnight in a supportive and collaborative environment.

Key Responsibilities
  • Develop and implement comprehensive care plans for clients receiving Home Care Packages, ensuring their clinical needs are met and exceeded.
  • Lead clinical governance and quality management frameworks within our services, upholding high standards of care and practice.
  • Provide direct and indirect clinical care to clients, applying advanced holistic care skills and expertise.
  • Contribute to the ongoing development of our program, participating in procedure development, planning, and reviewing care recipient services.
  • Maintain regular communication with the HCP Team Manager, reporting on KPIs and budget performance.
Requirements
  • Bachelor's degree in a relevant Aged Care, Community Services, Social Science, Nursing, or Allied Health discipline.
  • Demonstrated experience in home care packages, case management, and clinical knowledge, as well as expertise in community-based care and support services.
  • Knowledge of Aged Care funding streams and ability to manage budgets effectively.
  • Valid working rights in Australia, including a Police Check dated within the last three years.
  • Evidence of Covid-19 vaccinations and willingness to obtain annual flu vaccinations.
  • Own vehicle and driver's licence.
