Job Title: Case Manager - Homelessness Services
Job Summary:
Centacare North Queensland (NQ) is seeking a skilled Case Manager to join our Homelessness Services team. As a Case Manager, you will provide assertive outreach support to individuals and families who are homeless or at risk of becoming homeless.
Key Responsibilities:
- Provide case management support to individuals and families who are homeless or at risk of becoming homeless.
- Develop early intervention strategies to engage participants and achieve program and individual goals.
- Conduct initial assessments to ensure program eligibility and ongoing assessments to document participants' progress and barriers to achieving goals.
- Undertake outreach and community engagement activities as required.
- Contribute to program development and implementation, including planning and provision of community education and support.
- Utilize electronic data systems to record and maintain participants' personal information.
- Comply with legislation, service agreements, and Quality Management System requirements.
Requirements:
- A degree-level qualification in Social Work, Human Services, or Behavioural Sciences, or a Certificate level 4 or higher qualification in the community sector plus at least two years' experience working within a case management framework.
- Excellent written and verbal communication skills, as well as strong analytical skills.
- A current Driver's Licence.
- Currently hold, and/or eligibility to obtain and maintain, both a Disability Worker Screening and a Working with Children Blue Card.
Desirable:
- Experience in group facilitation.
- Experience working as part of a large and multi-disciplinary team.
- Willingness and ability to work outside normal office hours as determined by the Manager of Homelessness Services.
- Relevant experience in the Social or Community Service sectors or within a Not-For-Profit or Faith-Based organisation.
