Care Planning Specialist

The role requires a skilled registered nurse to establish, develop and evaluate holistic care plans for residents living in residential care homes.

This part-time position involves working 0.4FTE, Tuesday and Wednesday, at a Perry Park location.

As a Care Planning Specialist, you will promote quality of care through clinical assessments, involvement in care planning processes and collaboration with a multidisciplinary team.

Care Planning Responsibilities

  • Develop person-centred care plans
  • Coordinate specialist assessments and referrals
  • Perform ongoing reviews and address changing resident needs
  • Provide oversight of the ACFI/AN-ACC process and identify areas for improvement
  • Accept accountability for maximising funding and proactively engage with the multidisciplinary team to achieve best care and funding outcomes

About You

This role requires a registered nurse with current registration and a strong track record in delivering clinical and funding Key Performance Indicators (KPIs).

You must have demonstrated capacity to build effective relationships with key stakeholders.

Essential Requirements

  • Nursing qualification and current registered nurse registration with the relevant authority
  • Knowledge and understanding of aged care standards, legislation and regulations
  • Ability to support older people to live a good life
  • Flu vaccination and up-to-date COVID-19 vaccinations
  • Police Check for unsupervised contact/vulnerable groups less than 12 months old

Desirable Skills

  • Knowledge of continuous improvement methodologies
