We are seeking a highly skilled Assessment and Transition Coordinator to join our team. As an Assessment and Transition Coordinator, you will play a critical role in supporting the transition of prisoners back into their communities.
About the RoleThe successful candidate will have responsibility for assessing the transitional needs of prisoners, referring them to support programs and services, and assisting with discharge planning. They will also undertake risk and needs assessments, liaising with Community Correctional Services for parole planning and transition to Community Correctional Orders.
Key Responsibilities- Develop and coordinate prisoner risk assessment and pre-release services.
- Undertake prisoner risk assessments and needs assessments, contributing to the Case Management Review process.
- Support the delivery of the CV Reintegration Program at the prison location.
To be successful, you will have:
- Demonstrated understanding of female prisoners' transition and reintegration issues and needs.
- Able to administer risk assessment tools with individual prisoners.
- Capacity to work collaboratively with all stakeholders in a prison setting.
You will possess a tertiary qualification in an appropriate discipline, or demonstrated skills and experience in human services, welfare practice, or related fields. Experience using evidence-based approaches to engage complex clients is highly regarded.
