Job Overview:

As a Customer Care Manager, you will be responsible for managing a caseload of community-based older Australians with their Government funded aged care services delivered in their homes.

This role involves assessing clients' needs and developing personalised care plans, as well as collaborating with Clinical, Scheduling and Service Delivery teams to deliver comprehensive care.

You will work autonomously from home and within the community, offering flexible scheduling and opportunities for career development through training and education programs.

Key Responsibilities:
  • Manage client caseloads and develop care plans
  • Assess clients' needs and coordinate service delivery
  • Collaborate with multidisciplinary teams to achieve best outcomes
