Service Roster Coordinator
We are seeking a skilled and experienced professional to coordinate tailored services that enable individuals to live independently in their own homes.
About the Role
The Service Roster Coordinator will play a vital role in:
- Maximizing client autonomy through effective service coordination
- Supporting the rostering team to ensure the right care workers are matched with clients
Key Requirements
To be successful in this role, you will need:
- Minimum 1 year's experience in a similar role
- Able to provide positive solutions to complex problems
- A strong understanding of HCP, CHSP, TCP, or NDIS funding packages
- Able to work effectively as a team and independently
- Recent national police checks or willingness to obtain one
- Relevant tertiary qualifications in Aged Care or Disability
