Job Summary

We are seeking an experienced Registered Nurse to join our team as a Healthcare Specialist in Home Care. As a key member of our clinical team, you will provide high-quality patient care and support services to clients in their own homes, enabling them to live independently and maintain their health and wellbeing.

Key Responsibilities:
  • Deliver comprehensive clinical care and support services to clients in their homes, including developing personalized care plans and implementing effective treatment strategies.
  • Work collaboratively with senior clinicians, healthcare professionals, and other stakeholders to achieve optimal client outcomes.
  • Conduct thorough assessments of client needs and develop tailored care plans to address their physical, emotional, and social requirements.
  • Maintain accurate and detailed records of client interactions, care plans, and progress notes.
Requirements:
  • A relevant qualification in nursing and current AHPRA registration.
  • Previous experience as a Registered Nurse in a home care or community setting.
  • Valid working rights in Australia and a Victorian driver's licence.
  • A police check and flu vaccination (or willingness to obtain).

Location:

This role is based in Melbourne, Victoria, with regular travel required throughout the suburbs of Balaclava, Elwood, Ripponlea, St Kilda, Prahran, South Yarra, Windsor, Albert Park, Middle Park, Port Melbourne, and South Melbourne.
