Job Title: Disability Services ManagerOverview:

The Disability Services Manager plays a vital role in ensuring individuals receive personalized care that meets their unique needs. As a leader in client services management, this role will oversee the delivery of high-quality care plans, effective documentation management, and excellent communication with clients and stakeholders.

Key Responsibilities:
  • Client Relationship Management: Engage in client relationship management to ensure a high-quality customer experience.
  • Care Plan Review and Management: Ensure effective care plan review and management.
  • Leadership and Coaching: Lead and provide coaching to support workers in effective methods for delivering and exceeding client expectations.
  • Service Level Monitoring: Monitor service levels, including review of funding and client/carer satisfaction.
  • Consumer Support Plans and Assessments: Ensure consumer support plans and assessments are completed within required timeframes.
  • Case Conferences: Conduct case conferences with internal and external stakeholders in conjunction with the consumer and/or their representative when needs change or review of needs is required.
Requirements:
  • Community Services Coordination Experience: Demonstrated experience in a Community Services Coordination or Case Management role.
  • Disability Accommodation Management: Demonstrated experience in managing SIL/Disability accommodations.
  • Certificate IV in Community Services: Certificate IV in Community Services or relevant discipline (preferred).
  • Supervisory Experience: Demonstrated experience supervising, supporting, and training staff.
  • OH&S Knowledge: Knowledge of OH&S legislation relevant to aged care and disability service sectors.
  • Teamwork: Ability to work effectively as part of a multidisciplinary care team.
  • Communication Skills: High level of written and verbal communication skills.
