About The Role

A unique opportunity exists as a Clinical Nurse Specialist, to provide complex specialist nursing care and leading the development of specialist clinical practice and service delivery to ensure the provision of person centred, specialised, multidisciplinary care for people living with neurodegenerative conditions, including Dementia, Parkinson's Disease, Multiple Sclerosis, Muscular Dystrophy, Motor Neurone Disease and Huntington's Disease.

The provision of this service will be focused on care at home, in the community setting. As a telehealth enabled Health District, this role will provide care through a range on modalities including face to face, telephone and telehealth.
