Job Title: Integrated Care Coordinator

We are seeking a skilled ITC Care Coordinator to support our clients with chronic disease and complex needs.

This role involves working effectively with Indigenous outreach workers, arranging services outlined in the client management plan, ensuring transport arrangements, involving the client's family/carers as appropriate, and assisting clients to adhere to treatment programs.

The ideal candidate will have experience providing one-on-one support to clients, collecting data, writing reports, and client notes. A demonstrated understanding of the issues and barriers impacting Aboriginal and Torres Strait Islander people is also required.
