Complex care clinicians play a pivotal role in ensuring that individuals with complex needs receive comprehensive and coordinated support.

Job Description

The Complex Care Program is designed to address the unique challenges faced by consumers who require high levels of support. Our team works collaboratively with consumers, their families, and healthcare providers to develop personalized plans that meet their evolving needs.

  • We provide specialist clinical coordination, working closely with consumers and service providers to ensure seamless transitions and minimize duplication of services.
  • Our clinicians lead and coordinate complex discharge planning, streamlining the process and reducing the risk of hospital readmission.
  • Through strategic planning and negotiation, we actively engage with service providers to ensure accountability and foster collaborative problem-solving.

Key Responsibilities

  • Collaborate effectively within multidisciplinary teams to achieve optimal outcomes.
  • Contribute to quality improvement initiatives, including data collection and analysis to inform service development.
  • Maintain accurate and up-to-date documentation, adhering to established policies and procedures.

Required Qualifications and Skills

  • A minimum of 2 years post-qualification experience in mental health or a related field.
  • Registration with relevant health practitioner regulatory agencies (e.g., AHPRA).
  • Relevant postgraduate qualifications in mental health or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
