Job Title: Case Manager

About the Role:

  • Assess and investigate the needs of carers to provide them with tailored support and guidance.
  • Work collaboratively with carers, medical professionals, and other stakeholders to develop effective service plans that meet their needs and goals.
  • Provide emotional support and guidance to carers, helping them navigate complex systems and access available resources.
  • Think creatively and develop innovative solutions to overcome challenges and achieve positive outcomes for carers.

Requirements:

  • A degree in Social Work or a related field.
  • Experience working in a case management or support coordination role.
  • Strong understanding of the National Disability Insurance Scheme (NDIS) and My Aged Care.
  • Excellent communication and interpersonal skills.
  • Demonstrated ability to work collaboratively in a team environment.
