About the Role

We are seeking dedicated individuals to join our team as a Care Coordinator at Right at Home Southern NSW. As a key member of our team, you will be responsible for providing exceptional care and support to our clients in their homes.

Job Responsibilities
  • BUILDING STRONG CONNECTIONS WITH CLIENTS AND THEIR FAMILIES: Establish and maintain strong relationships with our clients and their loved ones, ensuring their needs are met and exceeded.
  • CLIENT CARE ASSESSMENTS AND CARE PLANNING: Conduct thorough assessments to identify client needs and develop personalized care plans that promote independence and well-being.
  • SUPPORTING CLIENT GOALS: Work closely with clients to understand their goals and aspirations, and provide support to help them achieve them.
  • Maintaining Accurate Records: Ensure accurate and timely recording of client information, adherence to Aged Care Quality Standards and NDIS Practice Standards.
  • Supporting Home Care Workers: Provide feedback and guidance to home care workers to ensure they have the necessary tools and resources to deliver high-quality care.
Essential Criteria
  • EXPERIENCE IN ASSESSING AND PLANNING CARE NEEDS: Proven experience in assessing and planning care needs, with a sound understanding of the Aged Care Quality Standards and NDIS Practice Standards.
  • SUPPORT WORKER EXPERIENCE: At least 3 years of experience working in the aged care or disability sector, with a focus on delivering person-centered care.
  • ORGANISATIONAL SKILLS: Strong organisational skills, with the ability to manage competing priorities and work effectively in a fast-paced environment.
  • ABILITY TO OBTAIN REQUIRED CHECKS: Ability to obtain a National Police Check, Blue Card, Drivers Licence, NDIS Worker Screening Check, First Aid & CPR Certificate.
