Job Title:

Supported Living Facilitator

Job Summary:

This role offers a unique opportunity to provide high-quality care and support to individuals in their own homes, promoting their independence and enhancing their quality of life.

Key Responsibilities:

  • Provide personal care and daily living assistance, including showering, personal hygiene, meal preparation, house cleaning, laundry, and ironing.
  • Support clients with community-based recreational and social activities, ensuring they remain connected and engaged with their communities.
  • Transport clients to appointments, shopping, and other destinations, using your own vehicle.
  • Implement individualized care plans, working closely with clients, families, and healthcare professionals to ensure seamless delivery of services.
  • Monitor and maintain accurate records of client interactions, progress, and any concerns or issues that may arise.

Required Skills and Qualifications:

  • Proven experience working in the disability, aged, or community sector, with a strong focus on person-centered care.
  • Excellent communication and interpersonal skills, enabling effective collaboration with clients, families, and multidisciplinary teams.
  • Ability to work independently, prioritize tasks, and manage multiple responsibilities while maintaining a high level of professionalism.
  • Current NDIS Worker Screening Check and CPR/First Aid Certification are mandatory for this role.
  • A valid Open Drivers License and access to a reliable vehicle are also essential.
